Appointments & Cancellation
If you need to cancel or reschedule your appointment, please do so at least 24 hours in advance to avoid being charged a missed appointment fee.
Wi-Fi is now available at all Allergy Asthma Clinic locations. Patients and their guests can access the Internet using their Wi-Fi-enabled laptops or tablets at no charge.
Allergy Asthma Clinic accepts most major health insurance plans except for AHCCCS (we do accept AHCCCS for foster children). See our medical insurance page for more information. Patients are responsible for verifying if their health plan is contracted by our practice.
All billing questions should be directed to our Billing Department at 602-277-3525. The office is open Monday through Friday from 9:00 am – 5:00 pm.
What to Bring
Please bring the following items to your first appointment:
- Insurance card(s)
- Driver’s license or other photo identification
- List of current medications and dosages
- Any pertinent medical records or test results
- Insurance Co-pay
- New patient forms
You can save time by printing and completing the new patient forms and bringing them with you to your first appointment.
Your privacy is considered a priority at Allergy Asthma Clinic. The HIPAA Notice of Privacy Practices describes how medical information may be used and disclosed.
